Personal Advisor – Leaving Care | Merseyside | Hybrid

Help young people leaving care take their next steps with confidence. As a Personal Advisor within Children's Social Care, you'll turn person-centred plans into practical support—from finding accommodation to accessing education, training and employment. This hybrid role in Merseyside lets you support care leavers as they build independence and shape their futures.

How you'll make a difference

• Help care leavers take practical steps towards suitable accommodation, financial capability, education, training and employment.
• Help assess needs and prepare, monitor and review Pathway Plans that put each young person at the centre.
• Stay in regular contact, supporting young people's wellbeing and confidence as they make decisions about their futures.
• Connect young people with community opportunities and specialist services that meet their needs.
• Identify risks and report safeguarding concerns through the appropriate channels.
• Work with families, council colleagues and partner agencies, including attending reviews and case conferences.
• Keep accurate, up-to-date case records on ICS and other systems, handle information confidentially and record authorised expenditure.

Assignment details

This is a temporary Personal Advisor assignment supporting young people leaving care. The role may include contributing to the Out of Hours Service. The team and office allocation may change following consultation.
