Hospital Team Manager

We are seeking an experienced Adult Social Care Team Manager to manage and develop our Hospital Discharge Team based at Charing Cross Hospital. This is a key management role overseeing a multidisciplinary team and ensuring the delivery of high-quality, person-centred services that promote well-being, choice and positive outcomes for residents. The successful candidate will provide operational management, supervision and guidance to Social Workers and other professionals, overseeing complex casework, safeguarding concerns and risk management.
