Reablement Care Coordinator

Belmont Recruitment are currently looking for an experienced Adult Social Care Practitioner to join Sheffield City Council's Reablement Team on an initial 6 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday. You will be responsible for assessing the needs of adults requiring social care support, developing individual care and support plans, and completing reviews to promote independence, wellbeing and positive outcomes.
