About this role
Our client Somerset Council is looking for an Adults Social Worker to join their Learning Disabilities team. Main Responsibilities and Duties Undertake detailed Care Act reviews and reassessments for adults with learning disabilities as part of the Review Project. Complete face-to-face visits with people in their homes, supported living services, residential settings and other community-based environments to gather evidence and assess current care and support needs.
