Adults Social Worker - hospital discharge to assess

Our client Manchester city council is looking for an Adults Social Worker to join their hospital discharge to assess team. Social worker required for a hospital discharge to assess community based team in South Manchester (Wythenshawe area). This is a fast paced, short term team who stay involved with citizens for up to 6 weeks. Our role is to complete full Care Act assessments within 7 days of discharge and then review the support in place within 6 weeks.
