Social Worker required for work within the Hospital Discharge Team in Bournemouth Competitive rates of pay rate up to £32.52. 37 hours per week. 3 month minimum contract with options to extend. Flexible, hybrid working arrangements. The post is to add assessment and review capacity to a high demand hospital discharge team based in the Bournemouth and Poole area. The role will be diverse but initially focus upon creating safe and effective discharge and flow from acute and community hospitals.
