Adults Hospital Discharge Social Worker Locum £38 an Hour 6 Month Initial Contract Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Alexander Hospital, Portsmouth 4 days a week and 1 day a week a working from home. Key Responsibilities: Assessment Duties: Complete Care Act assessments and Mental Capacity assessments . Make best interests decisions in line with the Care Act 2014 and Mental Capacity Act 2005 .
