Advanced Care Practitioner We are looking for experienced Advanced Care Practitioner (ACP) to join a busy, patient-focused primary care service delivering high-quality urgent and same-day care. This is an excellent opportunity for a motivated clinician who enjoys autonomous practice while working within a supportive multidisciplinary team. You'll assess, diagnose, treat and refer patients presenting with a wide range of undifferentiated and acute conditions, helping deliver safe, effective and timely care.
