Care Coordinator

ABOUT THE CARE COORDINATOR ROLE: As a Care Coordinator, you will play an integral part as one of the key people liaising between our Clients, Support Workers and Package Managers. We are looking for someone who is confident, capable and ready for a challenge. Cover shifts, on-call. You will need to demonstrate proven ability to coordinate people and systems and work as part of a team, supporting the Package Managers.
